What's the situation with Orton's injury?

WWE star Randy Orton has been on the shelf for most of the year, having last performed on the May 20 episode of Friday Night SmackDown, where he was written-off television after being attacked by The Bloodline (after RK-Bro lost the Raw Tag Team Titles to The Usos).
The Viper was spotted in Birmingham, Alabama yesterday, according to a report from PWInsider. WWE will often send its performers to Birmingham for surgery or to rehabilitate injuries.
Orton has been absent due to what is believed to be a serious back issue, though updates on his condition have been scarce.
Whether or not this is related, Randy's wife Kim Orton shared a post on her Instagram account celebrating the couple's anniversary, which included a picture of the fourteen-time world champion in a hospital bed and gown.
Kim did write 'if y'all only knew why this was taken', followed by a laughing emoji and the hashtag #insicknessandinhealth.
In late September it was reported that there was 'a lot of concern' within WWE regarding the severity of Orton's injury. The company had hoped he would be back in action by late 2022.
